Wernicke’s encephalopathy: A case report

We described a case of an alcoholic patient who presented with history Jaundice for 25 days, generalised weakness and loss appetite 20 days altered level consciousness 2 days. On examination, was grossly icteric there were no palpable lymphnode. Central nervous system examination revealed low GCS (E2V2M3), Pupil bilaterally pin point, Planter extensor abdominal showed mild hepatomegaly. MRI Brain symmetrical hyperintense areas on T2W FLAIR images seen in the Mamillary bodies, dorsomedial thalami, tectal plate, periaqueductal area around third ventricle. The lesions are hypointense T1W diffusion restriction DW images. These findings compatible Wernickes Encephalopathy. He started thiamine supplementation which neurological signs improved. Patient had RTA about 1 month prior to ICU admission. Then he underwent Spleenectomy operation. Again laparotomy operation done due intra-abdominal collection repair omental vessels thorough peritoneal collection.
